Supportive Housing Manager - Full-Time Days

Pay Range: $55,000 - $58,500/annually (Pay information will vary depending upon relevant experience for the position.)

The Supportive Housing Manager (Empire State Supportive Housing Initiative) is responsible for overseeing the day-to-day operations of services delivered at The Gardens of 270 East Avenue. Services include case management (Service Coordination), health and wellness programming, transportation to social outings and intake and admission coordination for 35 participants. The Supportive Housing Manager is responsible for ensuring all staff adhere to the guidelines and regulations from Department of Health related to the ESSHI manual as well as ensuring ESSHI (Empire State Supportive Housing Initiative) participants receive quality services.

ESSENTIAL JOB FUNCTIONS

  • Manage Service Coordinators to ensure that participant’s service plans and health and wellness assessments are current as per Department of Health guidelines.

  • Conduct weekly high risk resident meetings with Service Coordinators.

  • Provide training and orientation for newly hired staff to familiarize them with policies and procedures for the Supportive Housing Program and those of ESLC.

  • Implement ESLC’s professional conduct expectations that result in a positive community image and awareness of ESLC and the services that are provided.

  • Lead and collaborate with health and wellness coordinator to facilitate programming that will enrich the lives of the ESSHI participants.

  • Perform service coordination duties as needed.

  • Lead, manage and supervise Health and Wellness Coordinator, Health and Wellness Assistant, Service Coordinators and Driver.

  • Perform outreach to community referral agencies to inform of the program and to obtain referrals.

  • Schedule and conduct apartment tours for individuals interested in program enrollment.

  • Intake potential Supportive Housing residents and enroll them into the ESSHI program if appropriate.

  • Work with outside vendors to coordinate services based on the needs of the Supportive Housing participants.

  • Ensure Supportive Housing Participants receive quality services with high satisfaction through regular check-ins with the Participants

  • Conduct monthly staff meetings and staff supervisions (frequency dependent on employee needs) to ensure communication occurs and to hold employees accountable for stated goals and job expectations

  • Conduct quarterly Community Meetings to ensure Participants needs are being met and have a voice to be heard

  • Maintain staff schedule and time off request

  • Monitor, review and approval of time cards

  • Manage, review and approve credit cards purchases and allocations.

  • Educate partners and residents of the ESLC Neighborhood Program.

  • Responsible for a vibrant and successful Neighborhood Program at the Supportive Housing Program location.

  • Ensure all funder guidelines are followed

  • Ensure grant targets and deliverables are met

  • Actively manage the Supportive Housing program site budget to include meeting budget targets and adapt as needed

  • Review and approve appropriate invoices and expenses for the program site

  • Work with property management company to ensure Participant experience standards are met

  • Create new policies in collaboration with the Vice President of Community Services.

  • Provide continuous feedback including coaching to promote a high level of employee engagement and overall staff satisfaction.

  • Complete annual performance evaluations.

  • Collaborate with other ESLC team members as needed.

  • Perform other necessary duties as required. Employee may perform other related duties as assigned to meet the on-going needs of the organization.

Requirements

QUALIFICATIONS:

  • Current valid driver’s license.

  • Current health certificate with current PPD.

  • Flexible, yet organized, with the ability to exercise independent, sound judgment.

  • Must be knowledgeable about all community resources’, Medicaid Services, and available non-Medicaid services.

EDUCATION:

  • Bachelor’s degree required in a Health and Human Service field with three (3) years’ experience providing Care Management for individuals with experience homelessness, mental illness and/or Older Adults and knowledge about community resources.

EXPERIENCE:

  • Demonstrated ability to collaborate effectively in a team setting.

  • Must have experience working with homeless populations.

  • Ability to maintain effective and professional relationships with participants and other members of their care team.

  • Strong communication skills.

  • Ability to effectively engage clients in a therapeutic relationship, when appropriate.

  • Ability to work with Participants by telephone or in person.

  • Working knowledge of differential diagnosis of common mental health and/or substance use disorders, when appropriate.

  • Experience working with participants who have co-occurring mental health, substance abuse, and physical health problems.

  • Experience with community based and other social support services as well as healthcare services that respond to the individual’s needs and preferences and contribute to achieving the individual’s goals.

Performing duties of this job requires occasional walking and standing in and around the reception area, and lobby. Must be able to occasionally lift loads of 30 pounds without assistance and the ability to sit, talk, and hear is required. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, color vision, and the ability to adjust focus.

SUPERVISORY RELATIONSHIP:The work is performed within established guidelines and requires periodic supervision. The employee exercises initiative and independent judgment to effectively perform the essential job duties of this position.

RESPONSIBILITY FOR OTHERS:The employee has direct responsibility for Service Coordinators, Health & Wellness Coordinator, Health & Wellness Assistant and Driver.

SAFETY RESPONSIBILITIES:Shall be fully acquainted with all safety policies and procedures of ESLC. Takes an active role in all fire drills during the shift and/or emergencies for internal and external disasters as monitored by supervisor and incident manager. Must comply with facility in-service requirements.
