Bassett Healthcare Community Health Navigator II in Partial Remote, New York

Recently named a Forbes America's Best-In-State Employer 2022! What you'll do The Community Health Navigator II role provides quality care to Medicaid recipients eligible for Health Home services. In this role, a Navigator II is expected to assist members and coordinate members services with community resource, engage members in healthcare services, advocate for member’s needs, and assist members with reaching their person-centered goals. A Navigator II will work with High Risk members who are classified as HARP and/or Health Home Plus. High risk members require a higher level of care and coordination to ensure their needs are met. A Navigator is required to follow policy and procedure set by the lead Health Home to ensure quality and service alignment with NYS DOH guidelines. A Navigator must have good communication skills, time management and organization skills, have knowledge of and be able to collaborate with other community organizations, and work well with a team. It is expected that the Navigator II will maintain a caseload of 25 to 50 members at any given time, and assist with member coverage when the need arises. A Navigator II will work with the Community Health Navigation Supervisor to complete monthly quality reviews of enrolled member’s charts to ensure policy and procedure are being followed in accordance with DOH policy. This Navigator will complete spot checks in accordance with other Navigators trackers, paying close attention to annual requirements, uploads, HML’s, Care Plans and Assessments are complete and quality is aligned with policy requirements. Required to carry a caseload of a minimum of 30 members monthly while maintain quality according to the Bassett Health Home Policies. Caseload will consist of High-Risk HARP and HH+ members who require more intensive care management. Provide Core Care Management Services to assigned caseload. Comprehensive Care Management Care Coordination & Health Promotion Comprehensive Transitional Care Patient & Family Support Referral to Community & Social Support Services Completion of Care Plans, Assessments, updated documentation, home visits and billing. Complete outreach for assigned members per the HH Policy and Procedure, to enroll new members in the program. Providing education/guidance to patients and families on tools to manage chronic illnesses, develops individual and web-based tools and resources to improve compliance. Conducts thorough needs assessment and assist the member in setting goals and develops a service/care plan to address unmet needs. Develops, implements and monitors care plans with members and their families. Assists members engagement in their healthcare by connecting members with the appropriate medical services, closing care gaps, and ensuring transportation to medical appointments. Review monthly chart audits with Supervisor to ensure quality of charts. Attend required meetings to remain up-to-date on changes, and new DOH guidelines. Communication with Supervisor related to use of job duties and use of time off. Coordinate care through effective communication with other providers, community resources, and supports. Knowledge of County, State and Federal resources. Positive communication and schedule flexibility to appropriately support the needs of the team and the members being served. Collaborate with Bassett RN Care Managers for clinical oversight, as needed. Complete monthly member tracking on spreadsheets to ensure billing, requirements, and health and quality metrics are being met. Maintains current and accurate documentation of services provided to clients. Ensures all members information is entered into Medicaid Health Home data systems. Compliance with policy, procedure and regulatory requirements Meet minimum billing requirements for caseload by providing billable services as described in the Bassett Health Home policies and procedures. Attend webinars, and trainings as required by the DOH and the HH. Adheres to HIPAA confidentiality regulations 100% of the time as observed by manager. Assists with interviewing, and training new employees. Participate in rotating on-call activities. Assist with coverage of caseloads for other Navigators who are on Leave or Vacation to provide continuity of care. Assist in the development of resources and procedures that relate and impact job duties, as needed. Attends meetings and serves on committees, as requested. What we need from you Education: High School Diploma/GED, required. With an additional 6 yrs of experience providing direct services to people with Serious Mental Illness, Developmental Disabilities, alcohol and substance abuse. Associate's degree, preferred. With an additional 5 years of experience providing direct services to people with Serious Mental Illness, Developmental Disabilities, alcohol and substance abuse. Bachelor's degree, preferred, With an additional 2 years of experience providing direct services to people with Serious Mental Illness, Developmental Disabilities, alcohol and substance abuse. Degree Areas: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing Experience/Qualifications: 1 year experience as a Health Home care manager/Navigator serving SMI or SED population, required. 2 years in Human Services or Heathcare field providing direct care to individuals, preferred. Licensure/Certificates: NYS Driver's license, required. LPN Certification preferred. Must have behavioral health experience. CASAC Certification with 2 years of experience providing direct services to people with Mental Illness, Developmental Disabilities, alcohol and substance abuse, preferred. Skills: Time Management Computer Skills Self-Motivated Accountability Physical Requirements: Sedentary Work - Exerting up to 10 lbs of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Sedentary work involves sitting most of the time. Jobs are sedentary if walking or standing are required only occasionally, and all other sedentary criteria are met. Occasional: lifting, pulling, pushing, reaching, standing. Frequent: typing/ clerical/ dexterity, hearing, repetitive motion, seeing/ monitor/ computer use, talking, visual acuity (color, depth, perception), walking. Pay Range Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards.
