Intermountain Health Transplant Social Worker in Murray, Utah

Job Description:

This position provides comprehensive biopsychosocial assessments, treatment planning, brief counseling and support for patients and their loved ones through their transplant journey. They will participate in weekly multidisciplinary team meetings to provide recommendations, discuss appropriateness of a patient for transplant and how to overcome potential risks. The Transplant Social Worker will make community referrals based on recommendations and confirm patients complete the social work requirements.

Position Details:

  • Complete comprehensive assessments including: social, emotional, cultural, diagnostic impressions of mental health, cognition, financial, motivation, comprehension, and compliance

  • Provide education and transplant related resources to patients and their families

  • Manage and update patient social work information in electronic medical records system

  • Maintain a complete knowledge and participate in ongoing education to meet UNOS, OPTN and CMS regulations for Transplant Social Work

  • Care for and evaluate patients in multiple states

Shifts for this role are Monday-Friday from 8:30am-5:00 pm with occasional rotation of on-call weekend and holiday hours

The hourly range for this position is listed below. Actual hourly rate dependent upon experience and licensure.

Minimum Qualifications:

  • Master’s Degree in Social Work from an accredited institution by the Council on Social Work Education

  • Current Sate of Utah Licensed Clinical Social Work licensure or three years of post-MSW experience with mental health evaluations and care planning

Preferred Qualifications:

  • Transplant experience

  • Experience in working within multidisciplinary teams

  • Experience with end of life issues, crisis management, medical social work, mental health diagnostics

  • Experience working in inpatient and outpatient hospital settings

Physical Requirements:

  • Interact with others requiring the employee to communicate information.

  • Operate computers and other office equipment requiring the ability to move fingers and hands.

  • See and read computer monitors and documents.

  • Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.
