Description:

A member of the transplant multidisciplinary team. Conducts psychosocial diagnosis of transplant recipients, families, and social systems pre- and post-transplant. Develops treatment goals and empowers patients to remove barriers to ensure a successful transplantation experience. Provides brief, solution-focused counseling as needed, and facilitates a plan of care to ensure the patient’s psychosocial needs are addressed. Acts as a patient advocate. Provides referral to appropriate community resources to ensure continuity of care.

What a Transplant Social Worker will do:

  • Psychosocial assessment and psychosocial diagnosis- of living donors, transplant recipients pre- and post-transplant candidates

  • Psychosocial interventions, treatment planning, implementation, and coordination

  • Psychotherapy and/or counseling on issues related to:

  • Adjustment to disability and chronic illness

  • Adjustment to life-threatening or terminal illness

  • Adjustment to machine dependency

  • Adherence to one’s medical treatment plan

  • Preparation for the waiting period

  • Donors

  • Financial challenges

  • Rehabilitation, including physical, emotional, and social issues

  • Crisis intervention

  • Case planning, case management, resource management, care planning

  • Patient advocacy, resource brokering, and information and referral services

  • Continuity of care planning, including reassessment and evaluation

  • Admission diversion planning

  • Patient and family health education to maximize patient wellness and graft survival

  • Interdisciplinary collaboration and consultation

  • Documentation, report writing, and record keeping

  • Discharge planning and coordination

  • Program development and resource development

  • Clinical research

What Transplant Social Worker will need:

  • Master’s degree in social work or related field.

  • Licensed by the Michigan Board of Social Work as a Licensed Master’s Social Worker (LMSW), designated in clinical practice.

  • Preferred: 2 year’s prior Transplant or dialysis experience, and CCTSW certification

  • Computer competency in navigating Microsoft Office and other applications
